COVID-19 Survey with Gift Card for First 20 Respondents

NCUIH is inviting health care professionals at Urban Indian Organizations (UIOs) to take a 10–15-minute survey about COVID-19. This survey aims to identify existing barriers to COVID-19 vaccine confidence and educational needs related to COVID-19 management. The insights from the survey will support the development of tailored training materials and resources for UIOs. As a token of appreciation, the first 20 respondents to complete the survey will be eligible to receive a $25 gift card.
