One paragraph description:
The 2024 Alzheimer’s Grant Program, through Cooperative Agreements for Dementia Models of Care, aims to support the expansion and increase local clinical capacity for culturally relevant dementia care and services for American Indian and Alaska Native people living with dementia, their caregivers, and their communities. This program, titled “Addressing Dementia in Tribal and Urban Indian Communities: Enhancing Sustainable Models of Care,” is open to Federally recognized Indian Tribes, Tribal Organizations, and Urban Indian Organizations that are not existing awardees under the IHS Addressing Dementia in Indian Country program. Eligible applicants must provide ambulatory care services and coordinate with elder services. UIOs are encouraged to apply!

HRSA New Access Points (NAPs)

One paragraph description: HRSA announces that applications to establish New Access Points (NAPs) are now open to expand access to affordable, high-quality primary health care, including mental health and substance use disorder services, for underserved communities and populations. HRSA will only be able to make awards for this Notice of Funding Opportunity if Congress appropriates additional funds for the Health Center Program in fiscal year (FY) 2025. HRSA-funded health centers operate more than 15,000 service sites nationwide. If Congress appropriates additional funding in FY 2025, these NAPs will expand the number of health centers in the HRSA Health Center Program and help HRSA-funded health centers open new locations. NAP applicants may be currently funded health centers, look-alikes, or new organizations (those not yet part of the Health Center Program).
