The National Council of Urban Indian Health (NCUIH) invites Urban Indian Organizations (UIOs) to complete the following questionnaire on sexually transmitted infections (STIs) and sexually transmitted diseases (STDs). The purpose of this questionnaire is to improve NCUIH’s understanding of STI services, resources, and needs of UIOs. Findings from submitted questionnaires will be summarized in a report and shared with the Centers for Disease Control and Prevention (CDC) for program planning, development, and internal use. We hope to use these findings to inform future NCUIH trainings and opportunities, and to ultimately improve STI services at UIOs. No identifying information collected will be published, as any presentations of collected data will only be in aggregate.
Only one questionnaire can be submitted per UIO. Each UIO that submits a completed STI Questionnaire will receive a $250 gift card from NCUIH.
