With the flexibility of scheduling in mind, join us, for this exciting on-demand community of learning series which explores the need for, and approaches taken to integrate person-centered healthcare services into practice.
Session 1: Health Literacy an Essential Component of Communication Strategy for Person-Centered Care
Objectives:
- Understand the effects of low health literacy and how it translated into poor health outcomes
- Develop strategies to address health literacy in patient care to improve health outcomes
- Learn how to assess current health education materials and develop new patient centered educational materials with culturally relevant health literacy in mind

Session 2: Engaging Your Workforce and Healthcare Warriors to Improve Health Outcomes Through Person-Centered Care
Objectives:
- Understand the key elements of patient-centered care
- Identify what is being done around patient-centered care already in Native communities and why it matters in UIOs
- Identify strategies to empower patients to be active participants in health decisions
- Learn how a patient-centered care model can be an effective strategy in fostering trust between patient, provider and health care systems

Session 3: Navigating a Person-Centered Team Approach to Chronic Disease Management
Objectives:
- Understand how digital health technologies such as EHR can help improve medical practice management and increase patient health outcomes by increasing practice efficiencies and cost savings
- Discuss opportunities for frontline workers to navigate culturally relevant aspects of chronic disease management.
- Create quality coordination of care interventions that help the patient and referral staff schedule and close out of clinic services (integrated care meetings and mediation reconciliation/chart reviews)
